Create a Care Plan
To ensure all the bases are covered, collaborate with your healthcare team to create a personalized care plan. We understand that you might also need to see a specialist or other healthcare provider in addition to receiving care at NLFM. Communication amongst providers is critical to a good treatment plan. We’ll make sure to collect your treatment options, medications, dietary recommendations, and exercise routines all in one place, a cohesive care plan that you can feel confident about. With your permission, we also like to involve some family members in this process – this makes it easier to adhere to the plan together.
Make Notes
Once we have a plan in place, it’s up to you to stick to it. Keep track of your symptoms, medication schedules, and any side effects you experience. This information can be invaluable during appointments, helping us adjust your treatment as needed. There are convenient apps designed for this use (or try a simple journal).
